Skilled and Intermediate Care

The Lutheran Community Home is a 109-bed facility dually certified for both intermediate and skilled levels of care. We are fully licensed and Medicare certified.

Services include:

• Restorative care, wound care, IV therapy
• Physical, occupational & speech therapy
• Nutritional management
• On-site transportation, audiology, dental, optometry & podiatry services, laboratory & radiology services and beauty salon services
• Semi-private & private rooms available
• Assistance with Medicare, Medicaid and insurance provided

On-site activities and social events are also offered seven days a week.

Rehab to Home

Our skilled rehabilitation program offers a wide range of services, including:

• Orthopedic
• Cardiac
• Neurological
• Pulmonary
• Other medically complex conditions
  

Our staff includes professionals in Physical Therapy, Occupational Therapy, Speech Therapy, Nursing, Social Services, and Dietary Services. Our mission is to maximize functional independence and facilitate a safe discharge back home.

Secure Memory Care Unit

At our Secure Memory Care Unit, we offer assistance for older adults suffering from dementia or Alzheimer's Disease. We enable residents to stay active and maintain their independence as much as possible while benefiting from the attention provided by staff with specialized training in Alzheimer's care.
